Stage 1 — Begin With a Free Clinical Assessment From Home

Meet by secure video with a doctor of chiropractic who is also a neuromuscular soft-tissue shoulder specialist. The doctor reviews your symptoms, previous treatment and available imaging and observes your active forward flexion, lateral abduction, internal rotation and external rotation.

The assessment identifies whether your presentation appears consistent with adhesive capsulitis and whether you are suitable to continue toward ACM. You will also see procedure footage, animations and medical illustrations explaining the treatment and recovery program.

A shoulder X-ray is sufficient to begin. Upload an ultrasound or MRI when one is already available.

Stage 2 — Receive Your Personalized Plan

Patients who appear suitable receive their Toronto schedule and complete an anesthesia assessment. The clinical team confirms the imaging required, reviews your ability to complete the rehabilitation program and explains the complete itinerary before travel is booked.

The anesthesia team provides your fasting instructions and advises you about any medication adjustments required for IV sedation. Bring comfortable clothing that allows easy access to the shoulder and movement during the postoperative exercises.

Stage 3 — Complete the Final Toronto Assessment

Before Procedure Day, the clinical team completes the required physical assessment and program MRI. The orthopedic surgeon reviews the MRI and confirms the shoulder findings, while the anesthesia team completes the final clearance for IV sedation.

The orthopedic surgeon makes the final clinical candidacy decision, and you make the final decision about proceeding.

Stage 4 — Complete the Coordinated ACM Program

Procedure Day begins with active range-of-motion measurement using a digital inclinometer. The neuromuscular soft-tissue shoulder specialist then addresses protective muscle guarding, shortened tissues and weakened muscles before your active movement is measured again.

The contracted capsule is prepared under ultrasound guidance. Positioned on the operating table at our private hospital, you receive monitored IV sedation administered by a top anesthesiologist. You remain comfortable and breathe independently while the orthopedic surgeon performs controlled capsular mobilization with dedicated stabilization of the humeral head and shoulder joint.

ACM brings the muscular treatment, capsular preparation, surgeon-controlled mobilization and immediate rehabilitation together within one coordinated program.

Stage 5 — Begin Moving and Prepare to Travel Home

Stage 6 — Continue Your Recovery Wherever You Are

The home program begins intensively and gradually tapers as you maintain the restored movement, lengthen shortened tissues, strengthen weakened muscles and return to everyday activities.

Each focused lengthening and strengthening session generally takes about five minutes and can take longer depending on the patient and stage of recovery. The exercises require little or no equipment and can be completed at home, at work, in a hotel or while travelling.

The clinical team conducts weekly video follow-ups to review your active movement, exercise technique, strength, comfort and progress. Follow-up continues for up to twelve weeks, including when substantial function returns earlier.
